A Team Approach to Good Health
Our health education team is comprised of professionals dedicated to teaching people skills that promote lifelong health. Staffed with Certified Health Education Specialists, Registered Dietitians and Certified Diabetes Educators, the Health Education and Wellness Division is designed to provide programs dedicated to promoting lifelong wellness. The Health Education and Wellness Division offers a variety of health education classes and disease management programs.

Health education classes and disease management programs:

Asthma - Adult & Child/Caregiver - Blueprint for Breathing
This program is designed to help you better understand asthma, how it affects you and your family, and how to take control. Topics include triggers and treatments, medications and self-care.

Chronic Obstructive Pulmonary Disease (COPD) - Your Second Wind
Understand chronic breathing problems, how they affect you and your family, and how you can take control. This program is designed to help you manage your COPD and expose you to strategies and skills that will improve the quality of your daily living. Emphasis will be on nutrition, weight management, smoking cessation, physical activity, daily functioning, and monitoring progress.

Heart Failure (HF) – The Heart of the Matter
This program is designed to help you manage HF, and expose you to strategies and skills that will improve the quality of your daily living. Emphasis will be on nutrition, weight management, physical activity, daily functioning, and monitoring progress.

Diabetes - The Balancing Act
The art of balance is control; the key to control is knowledge. The Balancing Act is designed to provide you with basic knowledge of preventive care; healthy food and activity choices; blood glucose monitoring; foot and eye care; and stress management techniques.

Heart Health - Blood Pressure, Cholesterol, Triglycerides
You hold the key to your heart health! Recognize your risk factors for heart disease and high blood pressure, and find out what you can do about them. Learn to read and understand lab results, food labels, and restaurant menus. This program is designed to help you keep your heart health in control by reducing your weight, exercising, taking medications correctly, reducing tobacco and alcohol use, and eating from a balanced food plan.

Stop Smoking – Tobacco Cessation Program
With specialized training, our physicians and health educators will assist you in becoming tobacco-free. This program includes an evidence-based treatment plan, one-on-one consultation with a Tobacco Cessation Counselor, three months in a personalized behavior modification program, three months of personalized medication (depending on assessment), and member follow-up as needed. To be eligible, you must be a member of Health Plan of Nevada, Senior Dimensions, SmartChoice, or Nevada Check Up. Eligible members in the following categories will receive free medication and reduced fees for the program: adolescents, members with diabetes, cancer, chronic obstructive pulmonary disease, heart failure, parents of children with asthma, and those who are pregnant.

Weight Management – Adult – Lean on Me
See how good lean can look on YOU! Find the motivation to lose weight and keep it off. Develop a personal plan to make positive changes in your eating and exercise habits to achieve healthy and permanent weight loss. Identify your environmental and emotional triggers in order to take control of your habits and improve your self-esteem.

Weight Management - Children & Young Adults
An interactive program designed to help children and young adults improve their eating and exercise habits while building self-esteem and positive attitudes. This program emphasizes lifestyle, exercise, attitude and nutrition. Parent participation is required.
